Information in the NHS, better information means better care.

Sharing information in the NHS helps ensure that the quality and safety of services is consistent across the country. It can also highlight different diseases and conditions that may require more NHS investment. This is an opportunity for each of us to help the NHS provide high quality care for all.

You can find out more about how information is used for purposes beyond your direct care, the benefits this bring and the choices you have available to you by watching our short video. Home Visits

These are not encouraged and are assessed for clinical need or physically immobile. Requests should be phoned in before 10am, the Doctor could then arrange to see the most urgent cases first. The Doctor may telephone before visiting.

Please note that the Doctors will not visit if you move outside of our practice area.   Patients living in Chard town are advised to register with a local GP surgery should they require Home Visits.

Home Visits may be undertaken by our paramedics Mr Pat Riley, Mr Steve Mahoney or Ms Becky Guppy-Evans if the GP deems it suitable.

Our complex care Nurse Janet Haywood will arrange to visit you if you are housebound and require a review of your chronic disease, medication or care plan.
