Brendon also works with the CLICK PCN and his team will be available to do home visits for medication reconciliation for housebound patients who may be recently discharged from hospital or patients that have many medications and complex needs.

We will contact you to offer a visit by the pharmacy team when you have been discharged from hospital, your medication has changed or you are referred to the service by a GP.

Emma joined the practice in September 19 as a regular member of staff and GPs can refer you to her for 6 sessions of free NHS Counselling at The Meadows if they feel she is the most appropriate clinician.

Please note you cannot make an appointment without being refered by a GP, there will be a short waiting list and Emma will contact you directly.
